A: We accept most insurance, however if there is concern whether your insurance is accepted, please contact us before scheduling an appointment.

A: Check with your individual insurance provider/plan or call in advance.
Q: Are payment plans available?
A: Yes, we do make payment plans available for patients with high deductibles and co-insurance. We also accept all major credit cards. It is the patient's responsibility to make sure all medical insurance information is current and active at the time of your visit/treatment.

A: Insurance card, Picture I.D., A valid referral (if required), All previous medical records (if necessary), and a completed/updated patient form
